I, the undersigned cardholder ("Cardholder"), hereby authorize British Feed & Supplies ("Merchant") to charge the credit card identified on this form for payment of goods and services as follows:

1. Authorization Scope. If I have selected "This order only", I authorize a single charge for the order described in this form. If I have selected "Save card for future orders", I authorize Merchant to charge this card for the current order and for all subsequent orders placed under my account until I cancel this authorization in writing. Written cancellation requests must be delivered in person or by phone at (561) 633-6003 to the address above and will take effect within two (2) business days of receipt.

2. Processing Fee. I understand and agree that a 3.5% credit card processing fee will be added to each transaction total for orders processed through the GCAT program. This fee is non-refundable. Customers who choose to continue as direct British Feed & Supplies account holders outside of the GCAT program may be eligible for alternative payment arrangements at the sole discretion of British Feed & Supplies.

3. Statement Descriptor. Charges will appear on my credit card statement as "BRITISH FEED & SUPPLIES" or a similar merchant descriptor.

4. Accuracy of Information. I confirm that I am the authorized cardholder for the card identified on this form, that all information provided is accurate and complete, and that I have the legal authority to authorize these charges.

5. Refunds & Disputes. Returns and refunds will be processed in accordance with British Feed & Supplies' store policy in effect at the time of purchase. For billing disputes, I agree to contact British Feed & Supplies directly at (561) 633-6003 before initiating a chargeback with my card issuer.

6. Security. Card details provided on this form are used solely for processing authorized transactions. British Feed & Supplies does not store full card numbers on digital systems accessible via the internet.

7. Governing Law. This authorization is governed by the laws of the State of Florida. Any disputes arising from this authorization shall be subject to the jurisdiction of the courts of Palm Beach County, Florida.

8. Entire Agreement. This document constitutes the entire agreement between Cardholder and Merchant with respect to the authorization of credit card charges and supersedes any prior oral or written representations.

By signing below, Cardholder acknowledges having read, understood, and agreed to all terms set forth in this Credit Card Authorization Agreement.
